Terms and Conditions

1. CMC Kid’s Club (CMCKC) membership programme is available at CAH Medical Centres Sdn Bhd (CMC) (formerly known as Ramsay Sime Darby Health Care Sdn Bhd) Registration No.: 201301008653 (1038495-A) network of hospitals:
   • Ara Damansara Medical Centre – Ardee Club
   • Bukit Tinggi Medical Centre – Captain B Club
   • ParkCity Medical Centre – i-Kiddo Club
   • Subang Jaya Medical Centre – Teddy Paws Club

2. Membership is open to all kids below the age of 12.

3. Application must be made online through ADMC website.

4. CMCKC e-membership card is a privilege card which gives card holders the benefits of enjoying discounts, privileges, and events from time to time offered by CMC hospitals.

5. To enjoy the discounts and privileges, the CMCKC e-membership card must be presented during purchase and prior to making payment. No reimbursement or refund will be made on previous or back-dated transactions if card-holders fail to present the CMCKC card during registration.

6. Discounts are applicable for SELF-PAY customers upon full settlement of payment and are NOT applicable on doctor’s fees and other on-going promotions, discounts and / or privileges.

7. CMCKC e-membership card is non-transferable and is only for the exclusive use of the card-holder only.

8. There is no membership joining fee, and membership renewal is not required. CMCKC membership is valid until the child turns 12 years old on his/her birthday.

9. Room and board discounts are applicable for:

Hospital	Applicable for
Ara Damansara Medical Centre	Single Executive and Single bed room only
Bukit Tinggi Medical Centre	Single Deluxe, Single Standard, 2-bed room and 4-bed room only
ParkCity Medical Centre	Single Executive, Single, Double bed room only
Subang Jaya Medical Centre	Single Executive / Super Single, Single, 2-bed room and 4-bed room only

10. Upon submission of the online application form, an e-membership card will be issued to the applicant.

11. CMCKC benefits and privileges are redeemable during the next visit from the date of successful registration.

12. Members can enjoy the discounts and benefits across CMC hospitals.

13. Members are required to update CMCKC once their name has been changed from the mother’s to the baby’s name. Thereafter, an official e-membership card will be issued to the member.

14. If the card has been lost, stolen or damaged, kindly write in to inform CMC at admc.marketing@asia1health.com for replacement and CMC shall reserve its rights to impose a nominal replacement fee of RM10.00 per card.

15. For the benefits and privileges offered by the participating merchants (if any), CMC merely provides a platform to members to enjoy the privileges and benefits of the products/services of the said merchants. The relationship between CMC and the participating merchants does not create or constitute partnership, a joint venture, or an agency relationship and CMC shall not be held responsible and liable in any manner whatsoever for any implications of the products/services provided by the participating merchants. 16. Please allow up to 48 hours for a response to any correspondence. If a response has not been provided to you within 48 hours, please try contacting us again.

17. Should any of the conditions and privileges be misused or abused, CMC is entitled to suspend and terminate the card-holder’s right to use the CMCKC card without prior notification.

18. CMC reserves the right to terminate the programme, cancel and/or modify any of the CMCKC terms & conditions, benefits, and privileges without prior notice. Upon termination or cancellation, the CMCKC Card shall be nullified and invalidated and CMC shall have no obligation to extend any benefits and/or privileges under the programme.

19. CMC reserves the right to use and utilize all information obtained from or under or pertaining to this programme for marketing purposes and the card-holders shall be deemed to have consented to CMC of the same.

20. The card-holder shall notify CMC in writing at admc.marketing@asia1health.com of any updates or change of address or any other contact information to enable CMC to notify the card-holder of any information or updates on the programme.

21. The use of the card by the card-holder automatically infers acceptance of the prevailing terms and conditions of the programme.

22. Any information which the card-holder provides will be kept confidential and treated with care based on CMC’s privacy and personal data protection policy which is available on https://aradamansaramedicalcentre.com/privacy-and-pdpa-policy.

23. With the usage of the card, the card holder is regarded to have given his or her consent and authorizes CMC to collect, process, disclose and use his or her personal data within CMC Group for marketing purposes, except that disclosure will be made where required by applicable law or any governmental/regulatory authority or any valid legal process.

24. Each party shall comply with all applicable laws, statutes, regulations and each party’s Code of Business Conduct and policies relating to anti-bribery and anti-corruption throughout the term of this membership. In the event of any breach by a party, this shall be deemed as a material breach and the non-defaulting Party may, without prejudice to any other rights or remedies available, terminate this arrangement without any liability to the defaulting Party.
